Expert disclosure must be accompanied by a written report signed by the witness (if retained/specially employed or a party employee who regularly gives expert testimony). Report must contain: all opinions and bases; data considered; exhibits; witness qualifications including publications for last 10 years; list of cases with expert testimony in last four years; compensation statement.
